purchasing a shrine license plate for all of your vehicle
and motorcycles. Click on the "Request an Application" link
below to download and fill out the form. (Already printed
forms are also available from the Fraternal Office).
Send the completed form along with $10.00
to the Fraternal Office. The $10 goes to the Linn Van
Fund to purchase the Transportation Vans.
A stamped approved original will be returned to you. Take that to any BMV office
to get your license plate. BMV will charge a $15.00 service fee for the plate.
